Why Medical Weight Loss?

Around 40% of Singaporeans are classified as overweight (Health Promotion Board, National Population Health Survey), and obesity is complex — often shaped by genetics, environment, gut microbiome and lifestyle, not just willpower.

Medical weight loss takes a holistic approach. It combines structured lifestyle change with prescription medication when appropriate, and tracks progress through body composition rather than the bathroom scale alone. The result is gradual, sustainable progress rather than a crash that rebounds in 6 months.

Dr Joel has a Special Interest in obesity management, with the Basic Obesity Management Accreditation (BOMA) and Practical Obesity Management Skills (POMS) training. Each programme is built around your situation, not a one-size template.

The Programme

Why Choose Dr Joel's Weight Loss Programme

Holistic approach

A combination of medication, nutrition, exercise and lifestyle changes — designed for sustainable results, not a 30-day reset.

Special Interest in obesity management

Dr Joel has BOMA (Basic Obesity Management Accreditation) and POMS (Practical Obesity Management Skills) training to guide you through every stage of the weight loss journey.

In-clinic or telemedicine follow-up

See Dr Joel at the Jurong clinic or consult by telemedicine, with medication delivered to your home or collected at any of the four Singapore clinics.

Complimentary InBody scans

Body composition tracking is included with every follow-up so you can see fat loss versus muscle preservation, not just the number on the scale.

Why Weight Matters for Men's Health

Excess weight — particularly around the waist — drives many of the conditions men come to Dr Joel for. Treating the weight often improves several of these in parallel.

  • Cardiovascular risks — high blood pressure, high cholesterol, heart attacks and stroke are all closely linked to excess weight
  • Insulin resistance — visceral fat reduces the body's sensitivity to insulin, which raises blood sugar and increases the risk of type 2 diabetes
  • Fatty liver (MASLD) — metabolic dysfunction-associated steatotic liver disease (formerly non-alcoholic fatty liver disease), increasingly common in Singapore
  • Obstructive sleep apnoea — excess weight can obstruct the airway and restrict lung expansion, disrupting sleep and energy
  • Joint pain and reduced mobility — mechanical stress on weight-bearing joints, particularly the knees and hips, accelerates cartilage wear
  • Mental health and stigma — weight-related stigma and hormonal changes can affect mood, self-esteem and quality of life

Losing 5 to 10 per cent of body weight is often enough to meaningfully improve several of these.

What's Behind Weight Gain?

Weight gain rarely has a single cause. For most men, several factors stack up over years — and unwinding them takes more than a New Year's resolution.

Lifestyle and environment

  • Energy-dense convenience food — refined carbohydrates, sugary drinks, frequent dining out and late-night snacking
  • Sedentary work and long commutes — Singapore's white-collar work patterns make it hard to hit basic activity levels
  • Inadequate sleep — sleep deprivation increases appetite hormones (ghrelin) and decreases satiety (leptin)
  • Chronic stress — sustained cortisol promotes abdominal fat storage and emotional eating
  • Alcohol — high in liquid calories and disrupts sleep and metabolism

Genetics and metabolism

  • Genetic predisposition — a strong family history of obesity, type 2 diabetes or fatty liver shapes how easily weight is gained or lost
  • Insulin resistance — sets up a cycle of high insulin, fat storage and increased hunger
  • Gut microbiome — the balance of bacteria in the gut can influence how energy is absorbed and stored

Untangling which of these apply to you is part of the assessment. A man whose weight gain is mostly sleep and work stress needs a different plan from one whose drivers are largely metabolic.

What to Expect From an Assessment

The first consultation is about understanding why your weight has gone where it has, what's worth measuring, and what realistic targets look like for you. There is no judgement and no pressure to start medication.

What's measured

  • Body Mass Index (BMI) and waist circumference — establishing where you start
  • Body composition (InBody) — separates muscle, fat and water; useful as a baseline and to track progress over time
  • Blood pressure and resting heart rate

What's tested in the bloods

  • HbA1c, fasting glucose — to screen for prediabetes or type 2 diabetes
  • Fasting lipid panel — total, LDL, HDL cholesterol, triglycerides
  • Liver function tests — to check for fatty liver
  • Kidney function — baseline before any medication is started

The first visit is also when we set realistic targets. For most men, 5 to 10 per cent of starting weight in 6 months is a good first goal — meaningful for health, sustainable, and a foundation for further progress.

Treatment Options

Treatment is not "take medication and hope for the best." It is a sequence — built around lifestyle change, with medication added when the case justifies it.

1. Lifestyle changes (the foundation)

Lifestyle is the foundation of every plan, with or without medication. The aim is sustainable changes you can keep — not a 30-day challenge.

  • Nutrition — protein-forward, lower-refined-carbohydrate eating, with attention to portion size and meal timing rather than rigid calorie counting
  • Resistance training — 2 to 3 sessions a week of compound lifts to protect muscle mass during weight loss
  • Aerobic activity — 150 minutes a week of moderate activity (or 75 minutes vigorous) for cardiovascular and metabolic benefits
  • Sleep — aim for 7 to 8 hours; treat sleep apnoea where present
  • Stress management — cortisol drives abdominal fat; practical strategies matter more than just "relax"
  • Alcohol moderation — heavy drinking sabotages weight loss through liquid calories, sleep disruption and lowered self-control

2. Medication (when indicated)

Medication is considered when lifestyle changes alone don't reach the target, or when the starting BMI and comorbidities make medication a sensible first-line option alongside lifestyle. All weight loss medications in Singapore are prescription-only and require ongoing medical supervision.

  • GLP-1 receptor agonists — weekly or daily injections that reduce appetite, slow stomach emptying and improve blood sugar control. Examples include semaglutide (weekly), tirzepatide (weekly, dual GLP-1/GIP) and liraglutide (daily). Typical weight loss with consistent use: 10 to 20 per cent of starting weight over 12 to 18 months
  • Lipase inhibitors — oral medication that reduces dietary fat absorption; useful for some men, with predictable digestive side effects if fat intake is high
  • Appetite suppressants — short-term oral medication that reduces hunger and food preoccupation; used selectively and not long term

Side effects, monitoring requirements and contraindications differ between these classes. Dr Joel will explain the trade-offs clearly so you can decide what fits.

How the Weight Loss Programme Is Structured

Medical weight loss isn't a single visit — it's a programme, designed around regular check-ins and gradual progress. Here's what to expect from start to ongoing care.

Step 1 — Initial assessment

The first consultation covers your weight history, medical background, current medications, lifestyle and goals. Dr Joel runs a complimentary InBody body composition scan to establish your baseline — muscle mass, body fat percentage and visceral fat. Blood tests check for the metabolic factors that may be driving the weight (HbA1c, lipids, liver function, kidney function). Eligibility for medication is reviewed: GLP-1 receptor agonists are generally considered for adults aged 21 and above with a BMI of 30 or higher (obesity), or a BMI of 27 or higher with at least one weight-related condition such as type 2 diabetes, high blood pressure or sleep apnoea.

Step 2 — Personalised plan

Based on what the assessment shows, Dr Joel builds a plan around your situation rather than off a template. That includes nutrition guidance (adapted to your work and home routine), a structured movement plan with resistance training where possible, sleep and stress strategies, and — where indicated — prescription medication with a clear dosing and titration schedule. Everything is explained, with the trade-offs of each option spelled out so you can choose what fits.

Step 3 — Follow-up and progress tracking

Follow-up visits can be in person at the Jurong clinic or via telemedicine for convenience — particularly useful once you are settled on an ongoing prescription. Medication can be delivered or collected at any of the four Singapore clinics (Jurong, Tanjong Pagar, Orchard, Novena). A complimentary InBody scan is repeated every 3 to 6 months to track progress.

Why body composition matters more than the scale: weight loss almost always comes with some lean muscle loss, and without active monitoring, men can lose substantial muscle alongside fat. That lowers resting metabolism and increases the risk of regaining weight when treatment ends. Tracking muscle and visceral fat — not just total weight — is what tells us whether the programme is working in the way that lasts.

FAQ

Medical Weight Loss: Questions from Men in Singapore

Common questions Dr Joel Foo is asked about medical weight loss, GLP-1 medication and what realistic results look like.

What is medical weight loss?

Medical weight loss is a doctor-supervised programme that combines structured lifestyle change — nutrition, exercise, sleep, stress management — with prescription medication when appropriate. Unlike commercial weight loss programmes, the focus is on long-term metabolic health, not just kilograms on the scale, and it is tailored to your medical history, comorbidities and goals.

Am I a candidate for medical weight loss?

Most adults who have struggled to lose weight with diet and exercise alone, or who have weight-related health conditions, can benefit from a medical assessment. Prescription medication is generally considered for adults aged 21 and above with a BMI of 30 or above (obesity), or a BMI of 27 or above with at least one weight-related condition such as type 2 diabetes, high blood pressure or sleep apnoea. Dr Joel will work through your specific picture.

What BMI do I need to start weight loss medication?

Weight loss medication is generally considered for adults aged 21 and above with a BMI of 30 or higher (obesity), or a BMI of 27 or higher with at least one weight-related condition such as type 2 diabetes, high blood pressure or sleep apnoea. Body composition (waist circumference, fat distribution) and metabolic markers also factor into the decision — BMI alone is not the full story.

What are GLP-1 receptor agonists and how do they work?

GLP-1 receptor agonists are a class of medications originally developed for type 2 diabetes that mimic a natural gut hormone (glucagon-like peptide-1). They reduce appetite, slow stomach emptying and improve blood sugar control — typically resulting in 10 to 20 per cent weight loss over 12 to 18 months alongside lifestyle changes. They are given as a weekly injection or daily injection depending on the specific medication. They are prescription-only in Singapore and require medical supervision.

How much weight can I lose with medical weight loss?

Realistic targets are 5 to 10 per cent of starting body weight in the first 6 months with lifestyle changes alone, or 10 to 20 per cent with the addition of medication. Weight is lost gradually — typically 0.5 to 1 kilogram per week — and the goal is sustainable change, not rapid drops that come back. Some men respond more strongly to medication; others see most of their progress from lifestyle alone.

Are weight loss medications safe?

GLP-1 receptor agonists, lipase inhibitors and appetite suppressants are well-established and generally well-tolerated when used under medical supervision. Common side effects include nausea, indigestion and reduced appetite (which is partly the point). A proper medical assessment is needed before starting, and Dr Joel monitors progress and side effects throughout treatment.

Will I regain the weight if I stop the medication?

Some weight regain is common when medication is stopped, particularly if lifestyle habits weren't consolidated during treatment. The aim is to use medication as a tool — alongside changes in diet, exercise and sleep — to reach a healthier weight, then taper down or maintain a lower dose for long-term support. For men with severe obesity or strong genetic drivers, longer-term medication may be appropriate, similar to how blood pressure or cholesterol medications are taken long term.

Is weight loss medication covered by MediSave or insurance?

Coverage depends on your plan and whether the treatment is linked to an associated medical condition. It is best to check with your insurer before starting. Dr Joel's team can provide a clear cost breakdown so you can plan.

How much does medical weight loss cost in Singapore?

At Dr Joel Foo's Jurong clinic: consultation from $49.05, body composition analysis (InBody) complimentary for weight loss patients, oral appetite suppressants from $200 per month, and weight loss injections from $436 per pen. Final cost depends on the medication, dose and duration of treatment. WhatsApp 8095 3275 to confirm the latest rates and to plan a programme that fits your situation.
