What is Sildenafil?

Sildenafil is a tablet taken before sex to help with erectile dysfunction. It belongs to a class of medicines called PDE5 inhibitors, which improve blood flow to the penis by relaxing the smooth muscle in blood vessels. You may know it better as Viagra — that's the original brand name from Pfizer; sildenafil is the generic, which works the same way at the same doses but costs far less.

It was the first oral ED medication approved (US FDA, 1998) and is now one of the most studied medicines in the world. In Singapore it's a prescription-only medicine in 25 mg, 50 mg and 100 mg tablets — you'll need a consultation first so your doctor can check it's safe for you.

One thing to note: sildenafil is also prescribed at a different dose (20 mg three times daily, under the brand name Revatio) for pulmonary arterial hypertension. If you're already on it for that, don't take additional sildenafil for ED without your doctor's advice.

How Does Sildenafil Work?

Sildenafil works on the plumbing, not the desire. It helps blood flow into the penis when you're already aroused:

  • Blocks PDE5 (the enzyme that normally shuts down the erection signal too early)
  • Lets blood vessels relax so more blood can flow in when you're sexually stimulated
  • Helps you get and keep an erection — but only when you're actually aroused
  • Doesn't cause spontaneous erections — no stimulation, no effect

It also doesn't boost libido or sex drive. If the real issue is low desire or something more psychological, sildenafil alone won't fix it. That's why the consultation matters: Dr Joel will work out whether sildenafil is actually the right tool for what you're dealing with.

Is Sildenafil Right for Me?

Sildenafil works for most adult men with erectile dysfunction, especially when the underlying causes (diabetes, heart disease, low testosterone, stress) have been looked at and addressed. It's an on-demand medication — taken when needed, not every day — and not a cure for ED itself.

When sildenafil is NOT suitable

There are some situations where sildenafil can cause serious harm. Don't take it if you're in any of these categories:

  • On nitrate medication for heart disease (e.g. glyceryl trinitrate) — the combination can cause a dangerous drop in blood pressure
  • On riociguat or similar guanylate cyclase stimulators (used for pulmonary hypertension) — same risk of severe low blood pressure
  • Had a heart attack, stroke, or life-threatening heart rhythm problem in the last 6 months, or you have severe heart failure or unstable angina
  • Blood pressure is very low (below 90/50 mmHg) or very high and uncontrolled (above 170/110 mmHg)
  • Previous non-arteritic ischaemic optic neuropathy (NAION) — a rare eye condition affecting the optic nerve
  • You have a known allergy to sildenafil

When it needs careful discussion

Sildenafil can still be used in these situations, but your doctor will need to adjust the dose, watch for interactions, or consider alternatives:

  • You take alpha-blockers (for prostate enlargement or blood pressure) — timing and dose may need adjustment
  • You have liver or kidney disease — dose adjustment may be required
  • You take medications that change how sildenafil is processed — some antifungals, HIV medications, or certain antibiotics
  • You have Peyronie's disease, significant penile curvature, or cavernosal fibrosis — higher risk of priapism
  • You have retinitis pigmentosa — a rare inherited eye condition (use is generally not recommended)
  • Your heart condition makes sexual activity itself a concern

How Do I Take It?

Sildenafil is taken only when you need it — usually 30 to 60 minutes before sex — not every day. A few things affect how well it works:

  • You still need arousal — the tablet only works if you're sexually stimulated

Tips from Dr Joel

  • Give it 3 to 4 real attempts before deciding it doesn't work — performance anxiety can mask the effect on your first try
  • Don't combine with other ED tablets, or with sildenafil prescribed for pulmonary hypertension (Revatio)
  • Avoid grapefruit and grapefruit juice — they interfere with how your body breaks down sildenafil and can raise levels unpredictably
  • Go easy on alcohol — heavy drinking reduces effectiveness and increases side effects
  • Tell any other doctor or pharmacist you're on sildenafil before starting new medications
  • If the response isn't what you hoped for, Dr Joel can adjust the dose or switch approach at follow-up

Side Effects

Sildenafil is generally well-tolerated at standard doses. Most side effects are mild and pass within a few hours. A small number are serious and need urgent attention.

Common (usually mild, pass within hours)

  • Headache
  • Facial flushing
  • Indigestion or heartburn
  • Nasal congestion or runny nose
  • Dizziness
  • Back or muscle aches
  • Temporary visual changes — a bluish tint to vision, or increased sensitivity to light

Seek urgent medical help for:

  • Chest pain during or after sex
  • An erection lasting more than 4 hours (priapism) — this is an emergency and can cause permanent damage if left untreated
  • Sudden vision loss in one or both eyes
  • Sudden hearing loss, with or without ringing
  • Severe allergic reaction — rash, swelling, difficulty breathing

A few things worth knowing: sildenafil isn't a controlled substance in Singapore but stays prescription-only because of interactions. Many men use it safely for years with periodic review. It doesn't treat the underlying cause of ED, so if your overall health drifts (diabetes, heart disease, testosterone), effectiveness may too.

If side effects are bothering you, don't just tough it out — a lower dose or a different PDE5 inhibitor might suit you better. And don't take more than prescribed. Higher doses don't give a bigger effect; they just give more side effects.

How Long Does Sildenafil Last?
