What is a Testosterone Test?

A testosterone test is a simple blood test that measures your testosterone level — the main male sex hormone. It's used to check for low testosterone (hypogonadism), which can affect your sex drive, energy, mood, muscle mass and bones.

Low T is more common than many men realise — a Singapore primary care screening of 1,000 men found that around 26.4% had some form of androgen deficiency or low testosterone. Levels also naturally decline with age, typically 1–2% per year from your 30s onwards. If symptoms are affecting your life, a blood test is the first step to finding out what's going on.

Do You Have Any of These Symptoms?

Low testosterone doesn't always feel dramatic. It usually creeps in. If any of the following sound familiar, a blood test is worth considering:

  • Reduced sex drive (libido)

What Causes Low Testosterone?

Low testosterone rarely has just one cause. The big three categories:

  • Lifestyle and everyday health — being overweight, poorly controlled diabetes, chronic poor sleep, untreated sleep apnoea, heavy alcohol intake, and prolonged stress are by far the most common contributors. Past or ongoing anabolic steroid use also suppresses your body's own production
  • Testicular causes (primary) — the testes can't make enough testosterone, e.g. after chemotherapy or radiotherapy, testicular injury or surgery, a past bout of mumps orchitis, or genetic conditions like Klinefelter syndrome
  • Pituitary or hypothalamic causes (secondary) — the brain isn't signalling the testes properly, e.g. a pituitary disorder, long-term opioid or steroid use, iron overload (haemochromatosis), or chronic kidney or liver disease

Because there's usually more than one thing going on, Dr Joel will take a full history and may run other tests alongside the hormone panel — blood sugar, cholesterol, thyroid function — to give you the full picture.

What to Expect on Testing Day

The process is straightforward — no special preparation, no needles beyond a standard blood draw:

  1. Consultation — Dr Joel asks about your symptoms, medical history, lifestyle and any medications. This decides which tests are right for you.
  2. Morning blood draw — a small sample from a vein in your arm, ideally between 7am and 10am when testosterone levels are at their peak.
  3. Lab analysis — total testosterone results typically come back within 7 working days; extended panels (with free testosterone) can take up to 4 weeks.
  4. Follow-up — Dr Joel reviews the result with you, explains what it means in the context of your symptoms, and talks through next steps if any.

What's actually measured

The total testosterone level is the standard first test. If a fuller picture is needed, the panel can also include free testosterone (the biologically active portion, usually calculated from SHBG), and hormones like LH, FSH and prolactin that help pinpoint whether the cause is in the testes or the pituitary. Dr Joel will explain what's being ordered and why.

One important note: a single low reading isn't enough to diagnose low testosterone. If the first test comes back borderline or low, a second morning sample is usually needed to confirm it.

What Happens if My Testosterone is Low?

If your result comes back low and you have symptoms, the next step isn't automatically treatment. It's figuring out why. The goal is to help you feel better, not just to change a number on a report.

Lifestyle changes first

In most men, lifestyle and medical factors are driving the low reading — weight, sleep, alcohol, medications, chronic conditions. Addressing these can raise testosterone naturally over several months. Simple things that genuinely move the needle:

  • Regular resistance and aerobic exercise
  • Weight loss where it applies
  • Better sleep (target 7–9 hours; treat sleep apnoea if you have it)
  • Cutting back on alcohol
  • Managing stress

If a pituitary, thyroid or other medical issue is found, that's treated directly.

If lifestyle isn't enough: testosterone replacement therapy

Where hypogonadism is confirmed on repeat testing and symptoms are still affecting your life despite lifestyle changes, testosterone replacement therapy (TRT) may be discussed. TRT is given as an injection or a gel; it's a prescription-only treatment that needs baseline checks (haematocrit and, in older men, PSA) and ongoing monitoring. Why is the testosterone test done in the morning?

What counts as low testosterone?

Laboratory reference ranges vary. The Endocrine Society and the American Urological Association (AUA) generally consider total testosterone below approximately 300 ng/dL (10.4 nmol/L) to be low, while the CDC's harmonised standard uses a slightly lower threshold of around 264 ng/dL (9.2 nmol/L). A diagnosis of hypogonadism requires confirmation on two separate morning blood tests together with symptoms — the number alone is not enough. Dr Joel will interpret your result in context.

Should I get tested at a polyclinic or private clinic?

Polyclinics in Singapore offer subsidised blood tests for residents; hormone testing may be arranged through them, though referral pathways and scheduling can vary. Both options are valid; the right choice depends on cost, scheduling preferences, and whether you prefer a men's health-focused consultation.

How long do testosterone test results take?
