What Does the Hepatitis B Vaccine Protect Against?

Hepatitis B is a viral infection of the liver caused by the Hepatitis B virus (HBV). It is spread through blood, semen and other body fluids — via unprotected sex, sharing needles, needlestick injuries, or from mother to baby at birth. Unlike Hepatitis A, Hepatitis B can lead to chronic infection, particularly when contracted at a young age.

Acute Hepatitis B infection causes fever, fatigue, jaundice, abdominal discomfort and nausea, and most healthy adults clear the virus within a few months. However, about 5 to 10 per cent of adults (and a much higher proportion of infants and young children) go on to develop chronic Hepatitis B infection. Chronic infection can quietly cause liver inflammation, cirrhosis, and liver cancer (hepatocellular carcinoma) — often without symptoms until late-stage disease.

Hepatitis B is endemic in much of Asia, including parts of Southeast Asia. Singapore has substantially reduced new infections through its routine childhood vaccination programme, but adult catch-up remains important for those without immunity, those at risk of exposure, and travellers.

Should I Get the Hepatitis B Vaccine?

Under Singapore's National Childhood Immunisation Schedule (NCIS), all babies born in Singapore receive the Hepatitis B vaccine: the first dose at birth, with the course completed by 6 months as part of the routine 6-in-1 (hexavalent) vaccine. For adults, vaccination is recommended if any of the following apply:

  • You were not vaccinated as a child, or have no record and are unsure of your immunity status
  • You work in healthcare, laboratory or emergency services, or other roles with potential exposure to blood and body fluids
  • You live with or are sexually active with someone who has Hepatitis B
  • You have multiple sexual partners, men who have sex with men, or others at higher behavioural risk
  • You inject drugs or share injection equipment
  • You are on dialysis or are a recipient of regular blood products
  • You have diabetes mellitus — international guidance recommends Hepatitis B vaccination for adults with diabetes, who have a modestly higher risk of acute and chronic Hepatitis B
  • You have chronic liver disease, including chronic Hepatitis C, fatty liver disease, or unexplained persistently elevated liver enzymes (ALT / AST)
  • You were born in a country with high Hepatitis B prevalence (typically ≥2 per cent population prevalence — much of Sub-Saharan Africa, parts of Southeast Asia, the Pacific Islands, and parts of South America)
  • You are travelling to a country with high Hepatitis B prevalence, particularly for extended stays or if you may need medical or dental care during the trip

Internationally, the US ACIP has moved to a universal recommendation for Hepatitis B vaccination of all adults aged 19 to 59 (and adults 60+ at risk). Singapore's NAIS has not yet adopted universal adult vaccination, but the broad risk-based approach above captures most adults who would benefit.

If you are unsure whether you are already immune, a simple blood test can check your Hepatitis B surface antibody (anti-HBs), surface antigen (HBsAg) and core antibody (anti-HBc) status — this distinguishes prior vaccination, past infection, or chronic carrier status. Many adult Singaporeans completed the childhood vaccine series and are already immune.

Post-exposure prophylaxis (PEP)

If you have had a high-risk exposure to Hepatitis B — for example, a needlestick injury, sexual contact with a Hepatitis B carrier, or perinatal exposure — vaccination as part of post-exposure prophylaxis is recommended, ideally starting within 24 hours. Your doctor may also recommend a dose of Hepatitis B immunoglobulin (HBIG) alongside the vaccine in higher-risk scenarios (e.g., known infectious source, unvaccinated or known non-responder, newborns of HBsAg-positive mothers).

Hepatitis B Vaccine & Schedule

The Hepatitis B vaccines stocked at our Jurong clinic are non-live, recombinant vaccines — they contain a piece of the virus surface protein and cannot cause Hepatitis B infection. Both are registered with the HSA:

Engerix-B 20 Hepatitis B vaccine box by GSK — recombinant Hepatitis B vaccine used in Singapore
Engerix-B® 20 — recombinant Hepatitis B vaccine by GSK
Twinrix combined Hepatitis A and B vaccine box by GSK — inactivated Hep A + recombinant Hep B vaccine used in Singapore
Twinrix® — combined Hepatitis A and Hepatitis B vaccine by GSK

Engerix-B® 20

A standalone Hepatitis B vaccine by GSK for adults. The "20" refers to the 20-microgram adult antigen dose. It is the most widely used adult Hepatitis B vaccine in Singapore and is the one given under the NAIS catch-up programme.

Twinrix® (combined Hepatitis A + B)

A combined Hepatitis A and Hepatitis B vaccine, also by GSK. Useful if you need protection against both viruses — for example, before travel to a region where both are endemic. Twinrix uses a 3-dose schedule (0, 1 and 6 months for the standard adult course); an accelerated schedule is available if travel is imminent.

Other Hepatitis B vaccines used internationally

Newer Hepatitis B vaccines that may be available in some Singapore clinics include Heplisav-B® (a 2-dose adjuvanted recombinant vaccine by Dynavax, approved by US FDA in 2017) and PreHevbrio® (a 3-antigen recombinant vaccine by VBI Vaccines, approved 2021). Our Jurong clinic stocks Engerix-B and Twinrix as standard; if a 2-dose option such as Heplisav-B is more appropriate for you, your doctor can discuss availability or referral. Note that Heplisav-B and PreHevbrio are not currently recommended in pregnancy due to insufficient safety data, whereas Engerix-B has decades of pregnancy safety data.

Vaccination Schedule

Hepatitis B vaccination schedule
Option Schedule
Engerix-B® 20 (standard adult) 3 doses at 0, 1 and 6 months
Engerix-B® 20 (accelerated — for rapid protection) 3 doses at 0, 1 and 2 months, plus a booster at 12 months. Used when rapid protection is needed (e.g., imminent travel, urgent occupational exposure prep).
Twinrix® (combined Hep A + B, standard adult) 3 doses at 0, 1 and 6 months
Immunocompromised adults / dialysis patients Higher-dose schedule may be needed; post-vaccination antibody testing recommended — discuss with your doctor

Minimum dose intervals: at least 4 weeks between doses 1 and 2, at least 8 weeks between doses 2 and 3, and at least 16 weeks between doses 1 and 3. If your schedule is interrupted, you do not need to restart — complete the remaining doses as soon as possible.

Post-vaccination antibody testing (anti-HBs, measured 1 to 2 months after the final dose) is specifically recommended for healthcare workers, dialysis patients, immunocompromised adults, infants of HBsAg-positive mothers, and sexual / household contacts of Hepatitis B carriers. Most healthy adults outside these groups do not need routine post-vaccination testing.

Non-responders (anti-HBs < 10 mIU/mL after a full course) are typically offered a second 3-dose Engerix-B series. Adults who do not respond after two full courses are considered non-responders and may need workup or alternative strategies; discuss with your doctor.

The Hepatitis B vaccine can be safely co-administered with other vaccines (e.g. Hepatitis A, influenza, MMR) at the same visit — typically in different arms.

What to Expect at Your Visit

  1. Short consultation — your doctor reviews your medical history, allergies, any current illness, prior Hepatitis B vaccinations, and risk factors. Pre-vaccination antibody testing may be offered. Takes around 10–15 minutes.
  2. Vaccine administered — a single injection into the upper arm. Takes seconds.
  3. Brief observation — a short wait (typically 15 minutes) to check for any immediate reactions.
  4. Follow-up scheduled — our team will book your subsequent doses (1 month and 6 months after the first, for the standard schedule).

Common side effects

  • Soreness, redness or swelling at the injection site for 1–2 days
  • Mild fever, headache or tiredness
  • Occasional dizziness or feeling briefly faint
  • Mild muscle aches

Serious reactions are rare. Seek urgent medical help for signs of a severe allergic reaction (difficulty breathing, facial swelling) or a widespread itchy rash.

Precautions and contraindications

Let your doctor know if any of the following apply to you:

  • Severe allergic reaction to a previous Hepatitis B vaccine or any of its components
  • Severe yeast allergy — this is a contraindication to Engerix-B and Twinrix, as the Hepatitis B surface antigen is produced in yeast cells and trace yeast residuals may remain
  • Severe latex allergy — the tip cap of some prefilled syringe presentations may contain natural rubber latex; flag this so your doctor can confirm the presentation used
  • Active fever or moderate-to-severe illness on the day of the appointment — vaccination is usually deferred until you have recovered

Pregnancy and breastfeeding

Engerix-B and Twinrix are non-live recombinant vaccines and are considered safe during pregnancy and breastfeeding. Engerix-B is recommended for pregnant women who are at risk of Hepatitis B infection (for example, healthcare workers, partners of Hepatitis B carriers, or those with multiple sexual partners). Newer alternatives such as Heplisav-B and PreHevbrio are not currently recommended in pregnancy due to insufficient safety data.

FAQ

Hepatitis B Vaccine: Common Questions

What adults in Singapore most often ask about the Hepatitis B vaccine.

Who should get the Hepatitis B vaccine in Singapore?

Hepatitis B is part of Singapore's National Childhood Immunisation Schedule (NCIS), with the first dose given at birth and the course completed by 6 months. For adults, the vaccine is recommended for anyone aged 18 years or older who has not received prior vaccination or does not have evidence of past infection or immunity. Higher-risk groups include healthcare workers, household and sexual contacts of Hepatitis B carriers, people with multiple sexual partners, men who have sex with men, people who inject drugs, patients on dialysis, recipients of blood products, adults with diabetes mellitus, people born in countries with high (≥2 per cent) Hepatitis B prevalence, people with chronic liver disease or unexplained persistently elevated liver enzymes (ALT/AST), and travellers to areas where Hepatitis B is endemic. Internationally the US ACIP has moved to universal Hepatitis B vaccination for adults aged 19 to 59; Singapore's NAIS uses the risk-based approach above.

How many doses of the Hepatitis B vaccine do I need?

The standard adult schedule is 3 doses, given at 0, 1 and 6 months. An accelerated schedule (0, 1, 2 months, plus a booster at 12 months) is available if you need protection sooner — for example, before travel or starting an occupational role. Most adults achieve protective antibody levels after the 3-dose course and do not need further boosters unless you are immunocompromised or on dialysis.

Is there a combined Hepatitis A and B vaccine?

Yes — Twinrix is a combined Hepatitis A and B vaccine given on the same 3-dose schedule (0, 1 and 6 months). It is useful if you need protection against both viruses, particularly before travel. Your doctor will help you choose between standalone Engerix-B and Twinrix based on your past vaccination history and current needs.

Is the Hepatitis B vaccine MediSave-claimable in Singapore?

Yes. Hepatitis B is on Singapore's Nationally Recommended Vaccines list under the National Adult Immunisation Schedule (NAIS), and MediSave can be used by adult Singapore Citizens and Permanent Residents who have not previously been vaccinated or are not already immune — subject to annual withdrawal limits set by MOH. Children eligible under the National Childhood Immunisation Schedule (NCIS) are vaccinated at no cost as part of the routine programme. Our team will confirm your eligibility and process the claim at your visit.

Should I check my immunity before getting the Hepatitis B vaccine?

Pre-vaccination testing is optional but can be useful. A blood test for Hepatitis B surface antibody (anti-HBs), surface antigen (HBsAg) and core antibody (anti-HBc) can confirm whether you are already immune (from prior vaccination or past infection) or whether you are a chronic Hepatitis B carrier. Many Singaporeans vaccinated under the childhood programme are already immune. If your antibody level has waned but you completed the original course, current guidance is generally that no booster is needed in immunocompetent adults.

What are the side effects of the Hepatitis B vaccine?

Most side effects are mild and pass within a day or two — soreness, redness or swelling at the injection site, mild fever, headache, fatigue, or occasional dizziness. Serious reactions are rare. Seek urgent medical help for signs of a severe allergic reaction (difficulty breathing, facial swelling) or a widespread rash. The Hepatitis B vaccine has been used safely for decades in Singapore's childhood programme.

Can I get the Hepatitis B vaccine while pregnant or breastfeeding?

Yes. The Hepatitis B vaccine is a non-live recombinant vaccine and is considered safe during pregnancy and breastfeeding. It is recommended for pregnant women who are at risk of Hepatitis B infection (for example, healthcare workers, partners of Hepatitis B carriers, or those with multiple sexual partners). Your doctor will discuss the timing with you at your visit.

How much does the Hepatitis B vaccine cost in Singapore?

At our Jurong clinic, Engerix-B (Hepatitis B only) is $54.50 per dose and Twinrix (combined Hepatitis A and B) is $125.35 per dose. Three doses are needed for the standard adult course. Eligible Singapore Citizens and Permanent Residents may use MediSave for Hepatitis B vaccination under the NAIS. Prices are NETT and inclusive of GST; consultation fees are charged separately.
