An allergy is an immune reaction to something harmless. On first exposure the immune system makes an antibody called immunoglobulin E (IgE) against that substance; on later exposures the IgE triggers the release of histamine and other mediators, producing the familiar symptoms. Allergy testing looks for that IgE — either by measuring it in the blood, or by provoking a controlled reaction in the skin.
This is worth doing because allergic disease is common here. House dust mite sensitisation accounts for a large share of allergic symptoms in Singapore and is found in the majority of asthma patients, according to a review of the local allergen landscape published in the Asia Pacific Allergy literature. Allergic rhinitis is estimated to affect more than 30% of people in Singapore, most of them children and young adults, according to Singapore’s Agency for Care Effectiveness.
Food allergy is far less common locally — fewer than 3% of children in Singapore’s GUSTO birth cohort had one in their first four years — so testing is often as useful for ruling a food out as for confirming it.
Allergic symptoms cluster by the route of exposure — airborne triggers tend to affect the nose, eyes and chest, while foods and drugs more often affect the skin and gut.
Sneezing, a persistently blocked or runny nose, post-nasal drip, and itchy or watery eyes. Cough, wheeze or breathlessness — often worse at night or on waking. Symptoms that flare in bed or while cleaning point towards dust mite.
Hives (urticaria) — raised, itchy welts that come and go. Eczema that flares without an obvious cause or is difficult to control. Swelling of the lips, eyelids or face, known as angioedema.
Abdominal pain, vomiting or diarrhoea shortly after a particular food. Itching or tingling in the mouth when eating certain fruits or nuts — often a sign of cross-reactivity with pollen rather than a true food allergy.
Light-headedness, throat tightness or difficulty breathing after an exposure. These are not symptoms to test your way through — they need emergency care first, and testing afterwards.
Our climate shapes the local allergen profile. Warm, humid conditions year-round favour dust mites and moulds, and there is no pollen season in the temperate sense.
House dust mite is the single most important inhalant allergen in Singapore — predominantly Blomia tropicalis, Dermatophagoides pteronyssinus and Dermatophagoides farinae. Cockroach, cat and dog dander, and mould spores such as Aspergillus and Cladosporium follow. Tropical tree pollens including Acacia also feature.
Shellfish — prawn, crab and related species — is the most frequent food trigger in Singaporean adults. Egg and cow's milk dominate in young children, with peanut and tree nuts important at all ages because they more often cause severe reactions. Food is the leading cause of anaphylaxis presenting to emergency departments locally.
Reactions of this kind are immune-driven and fast. Symptoms that build over hours and stay in the gut — bloating, wind, loose stools — are more often a food intolerance, which works by a different mechanism and is not detected by an allergy test.
Both look for the same thing — IgE directed at a specific allergen — by different routes, and neither is universally better.
A specific IgE blood test is one draw, measured in the laboratory. It is unaffected by antihistamines or by the state of your skin, covers many allergens at once, and carries no risk of provoking a reaction, which makes it the safer choice after a severe one. Results take longer.
A skin prick test places a drop of allergen extract on the forearm and pricks the surface; a raised wheal within 15–20 minutes indicates sensitisation. You get results in the same visit and the cost per allergen is lower, but antihistamines must be stopped for several days beforehand and you need clear skin to test on — which rules it out for some people with extensive eczema.
A conventional test measures your reaction to a whole allergen — "peanut", say — as a single unit. Molecular testing, also called component-resolved diagnostics, measures the reaction to the individual proteins inside it.
That distinction is clinically useful. Within one food, some proteins are stable and associated with systemic reactions, while others are fragile, destroyed by cooking, and mainly responsible for cross-reactivity — the reason someone allergic to birch pollen may get an itchy mouth from apple without being meaningfully allergic to apple. Testing at component level helps separate a genuine allergy from a cross-reaction, which changes how strict avoidance needs to be.
No fasting is needed. If a skin prick test is planned, antihistamines are usually stopped for a week beforehand — check with us rather than stopping anything on your own. Antihistamines do not affect blood testing.
You will see a doctor first to go through your symptoms and decide what to test. A blood draw takes a few minutes and most patients are done within 30–60 minutes outside peak hours; skin prick testing adds a 15–20 minute wait while the reaction develops. Skin prick results are read in the same visit, blood results generally take 1–2 weeks, and the 300 allergen molecular panel is usually back within about 3 working days.
Specific IgE results are reported as a concentration and usually graded into classes, from undetectable up to very high. A higher class means more IgE is present, which broadly correlates with a greater likelihood of a genuine clinical reaction — but it does not directly measure how severe a reaction would be.
The important caveat is that sensitisation is not the same as allergy. It is entirely possible to have detectable IgE to something you eat without trouble. A result is only meaningful read against your history: what you were exposed to, how quickly symptoms appeared, and whether it has happened more than once. A positive result that does not fit your story usually needs discussion rather than blanket avoidance — unnecessary exclusions carry their own costs, particularly for children.
Where results and history do line up, the outcome is practical: a clear list of what to avoid, what is likely safe, and how to treat symptoms when exposure happens anyway.

The difference is breadth — and, in the 300 panel, depth, because many entries are tested at molecular level rather than as a whole extract. A skin prick test places a drop of allergen extract on the skin and pricks the surface; a raised wheal within about 15–20 minutes suggests sensitisation. A blood test measures specific IgE antibodies to an allergen in a blood sample.
Skin prick gives same-visit results and is inexpensive per allergen, but needs antihistamines stopped beforehand and clear skin at the test site. Blood testing is unaffected by antihistamines or eczema, tests many allergens from one draw, and is preferred if you have had a severe reaction. Your doctor will advise which is more suitable.
For skin prick testing, yes — antihistamines suppress the skin response and can produce a false negative, so we ask you to stop them for a week beforehand. The interval that matters clinically is nearer 3–7 days depending on the medicine, but a week covers all of them.
For allergy blood testing, no. Specific IgE is measured in the blood and antihistamines do not affect the result, which is one reason blood testing is chosen when someone cannot come off their medication. Never stop a prescribed medicine without checking with your doctor first.

House dust mite is the dominant one in our climate and is linked to a large share of allergic symptoms locally. Allergic rhinitis is very common — Singapore’s Agency for Care Effectiveness estimates it affects more than 30% of the population, most of them children and young adults. Other frequent triggers are cat and dog dander, cockroach, mould spores, and tropical pollens such as Acacia.
Among foods, shellfish, egg, cow’s milk, peanut and tree nuts account for most reactions, though food allergy is far less common here than airborne allergy — fewer than 3% of children in Singapore’s GUSTO birth cohort had one in their first four years.
Standard testing measures your response to a whole allergen extract. Molecular testing, also called component-resolved diagnostics, measures the response to individual proteins within that allergen.
This matters because some proteins are strongly associated with severe reactions while others mainly cause cross-reactivity — the reason a birch pollen allergy can make apple feel itchy. Knowing which component you react to helps separate a genuine food allergy from cross-reactivity, so avoidance advice can be narrower and more accurate. The 300 allergen panel combines 218 molecular components with 82 whole extracts from a single blood sample.
Not on its own. A positive result shows sensitisation — your immune system has made IgE against that allergen — but some people are sensitised without ever reacting to it.
An allergy diagnosis is made by reading the result together with your symptom history: what happened, how soon after exposure, and how reproducible it is. This is why testing follows a consultation rather than replacing one, and why testing a very wide panel without a clinical reason can produce findings that are hard to act on.
Yes. Both blood and skin prick testing can be done in children, and a Paediatric / Common Allergy Panel covering 15 frequent childhood triggers is available. Blood testing is often easier in younger children or where eczema covers the usual skin test sites.
Food allergy is most common in early childhood, and many children outgrow milk and egg allergy, so repeat testing over time is sometimes advised to see whether a diagnosis still stands.

Management usually combines trigger avoidance with treatment of the symptoms — antihistamines, nasal corticosteroid sprays for allergic rhinitis, or emollients and topical treatment for eczema. Where reactions have been severe, an adrenaline auto-injector and a written action plan may be advised, and referral for immunotherapy can be discussed.
