What Is a Homocysteine Test?

Homocysteine is an amino acid — a building block your body produces as it breaks down protein from your diet. Normally it does not hang around: with the help of three B vitamins (folate, vitamin B12 and vitamin B6) it is quickly converted into other, harmless substances. The homocysteine blood test measures how much is circulating in your blood.

When that conversion is impaired — most often because those B vitamins are low, or because of an inherited variation in a gene called MTHFR — homocysteine builds up. A raised level (hyperhomocysteinaemia) is therefore useful in two ways: it can flag a hidden B-vitamin deficiency, and it is associated with a higher risk of heart attack, stroke and blood clots.

It is important to be clear-eyed about what the test does and does not tell you. Homocysteine is a marker, read alongside your cholesterol, blood pressure and other risk factors — not a stand-alone diagnosis. And as the sections below explain, whether homocysteine directly harms arteries or simply reflects other problems is still debated. That nuance shapes how a raised result is used.

Why a High Homocysteine Matters

A raised homocysteine has been linked in many studies with a higher risk of:

  • Heart attack and coronary artery disease
  • Stroke and other cerebrovascular disease
  • Peripheral artery disease — narrowed arteries in the legs
  • Venous blood clots (deep vein thrombosis and pulmonary embolism)

But there is an important caveat. This is an association, and association is not proof of cause. It is debated whether a high homocysteine actively damages blood vessels, or whether it is mainly a marker of something else — such as low B-vitamin intake, reduced kidney function or an underlying genetic difference — that carries the real risk. Studies that lowered homocysteine with vitamins have generally not reduced heart attacks or strokes, which points towards it being more marker than driver.

That does not make the test pointless — far from it. A raised homocysteine is a genuinely useful prompt: to check and correct B-vitamin status, to look for other causes, and to take your overall cardiovascular risk seriously. It is simply interpreted with that context in mind.

Who Should Have a Homocysteine Test?

Homocysteine is not a routine test for everyone. It is used selectively, and is most worth considering if you have:

  • A personal or family history of early cardiovascular disease — heart attack or stroke at a young age, particularly without the usual risk factors.
  • Unexplained blood clots — a deep vein thrombosis or pulmonary embolism, especially when recurrent or without a clear trigger.
  • A suspected B-vitamin deficiency — low folate or vitamin B12, which can also cause tiredness, nerve symptoms or anaemia.
  • A known or suspected MTHFR gene variant in yourself or the family.
  • Recurrent pregnancy loss — where raised homocysteine is sometimes investigated as one possible factor.
  • A wish to complete a cardiovascular risk picture — often alongside markers such as hs-CRP and lipoprotein(a) as part of a cardiac screen.

Understanding Your Homocysteine Result

What Causes a High Homocysteine?

Because homocysteine sits at a crossroads of nutrition, genetics and organ function, several things can push it up:

  • Low B vitamins — deficiency of folate, vitamin B12 or vitamin B6 is the most common and most treatable cause.
  • MTHFR gene variants — a common inherited difference that reduces how efficiently folate is processed.
  • Reduced kidney function — the kidneys help clear homocysteine, so impaired function raises it.
  • An underactive thyroid (hypothyroidism).
  • Lifestyle factors — smoking, heavy alcohol intake, and a diet low in vegetables and wholegrains.
  • Certain medications and increasing age.

What to Do If Your Homocysteine Is High

The first job is to find the cause. Since low B vitamins are the most common and most correctable reason, your doctor will usually check your folate, vitamin B12 and vitamin B6 status, and consider kidney and thyroid function too. Depending on what is found, steps may include:

  • Correcting a B-vitamin deficiency — through diet (leafy greens, legumes, wholegrains and B12 sources) and, where needed, supplements, which reliably bring the level down.
  • Treating an underlying condition — such as an underactive thyroid or reduced kidney function.
  • Lifestyle changes — stopping smoking and moderating alcohol, both of which raise homocysteine.
  • Managing your overall cardiovascular risk — cholesterol, blood pressure and blood sugar, since these matter regardless of the homocysteine number.

One honest caveat worth repeating: while B vitamins reliably lower homocysteine, large clinical trials have shown that this does not translate into fewer heart attacks or strokes.
